This free printable emergency information sheet is the perfect addition to your personalized first aid kit.
If you’ve ever experienced an emergency with one of your children, you understand the importance of being prepared.
Our daughter Madison has quite a few allergies, and also suffers from asthma. It only took one asthma attack without any medicine (before her diagnosis) for us to realize the value of being prepared. We decided to put together a compact first aid kit tailored specifically to her needs. We never leave home without it.
Our custom first aid kit has everything she might need while away from home. Inhaler plus spacer, Benadryl cream and tablets, allergy medication, and tissues.
To store these items, we went with the Oh Joy! first aid bag by Band-Aid. It’s compact, yet fits everything we need. Plus, it has a durable exterior that is also super cute! Who said you can’t add a little sprinkle of cuteness to your first aid kit?
As you can tell, our kit is setup specifically for an asthma attack or allergic reaction.
I also keep an emergency information sheet in her first aid kit.
We’ve all filled these out for school and for rec sports, but how many times do we include this form with us day-to-day, when the majority of these emergencies actually occur?
Sheet information
I’ve designed the sheet to fit all of your standard emergency information, including:
- Doctor’s contact information
- Insurance policy number
- Family emergency contact phone numbers
- List of known allergies
I also include details for the medications stored in her first aid kit, along with directions on how to administer each.
This would also be the perfect information sheet to leave with your nanny or babysitter.

Before becoming a mommy, I worked in EMS as an emergency medical technician. I responded to countless calls where children and parents were faced with a medical emergency. Many of these could have easily been prevented with a simple first aid kit tailored specifically to a child’s known needs. It takes only a few minutes to assemble but can mean the world when it really counts.
Let’s be sure to enjoy summer and stay safe at the same time!
